Primaire gelokaliseerde cutane nodulaire amyloidosis (
primary localized cutaneous nodular amyloidosis (PLCNA))
van de voet is erg zeldzaam. Er zijn slechts enkele case reports beschreven. De laesies kunnen aan de onderkant van de voet zitten, vooral rond de tenen of tussen de tenen in de interdigitale ruimte. Soms wordt het voorafgegaan of vergezeld van blaren. Het wordt veroorzaakt door een ophoping van
AL-amyloid (light-chain amyloid) in de reticulaire dermis. De oorzaak is niet bekend. Mogelijk is het een reactie op repeterend trauma. Of een lokaal plasmacytoma. Het is geen uiting van systemische amyloidosis. In een case report wordt wel geadviseerd om dit uit te sluiten (consult hematoloog). Progressie naar systemische amyloidose wordt niet verwacht. In het verleden is dat wel eens beschreven maar bij nadere analyse van dat case report bleek het vanaf het begin te gaan om een cutane lokalisatie van systemische amyloidose.

Cutane amyloidosis is zeldzaam en wordt onderverdeeld in
maculair,
lichenoid, en
nodulair. Maculaire amyloidosis en lichen amyloidosis vormen een spectrum en kunnen gelijktijdig voorkomen. De amyloid depositie is afkomstig van keratine en ontstaat mogelijk door chronische beschadiging van de epidermis.
Nodulaire cutane amyloidosis (nodular localized cutaneous amyloidosis) is zeer zeldzaam. Het amyloid materiaal is niet afkomstig van keratine zoals bij de andere vormen van cutane amyloidose, maar het is AL-amyloid afkomstig van lichte ketens immunoglobulinen, geproduceerd door een lokaal infiltraat van plasmacellen. Het is niet bekend waarom die plasmacellen zich lokaal ophopen in de huid. De infiltraten zijn polyclonaal, maar soms monoclonaal. Toch gaat het niet om een plasmacel maligniteit, per definitie is het benigne en beperkt tot de huid. Het kan geassocieerd zijn met auto-immuunziekten, vooral Sjögren syndroom, ook CREST, RA, SLE en primaire biliaire cirrhose.

Diagnostiek:Biopt. Biopt buikvet om systemische amyloidoses uit te sluiten. M-proteine screening.
Hb, Leuko’s, leukodiff. Kreat, ALAT, ASAT, gamma-GT, AF. Consult hematoloog.
Therapie:
curettage en electrocoagulatie.
CO2 laser ablatie.
excisie, shave-excisie
cryotherapie.
