FACIAL DISCOID DERMATOSIS home ICD10: L30.9

Facial discoid dermatosis (persistent facial discoid dermatosis) is een huidafwijking in het gelaat bestaande uit oppervlakkige ronde, soms annulaire erythemateuze, roze of oranjekleurige schilferende papels en plaques. De individuele laesies lijken wat op seborroïsch eczeem maar de distributie is anders. Het bijzondere aan deze afwijking is dat de laesies statisch zijn, dat wil zeggen ze ontstaan op bepaalde plekken in het gelaat, meestal op de wangen, de kin of het voorhoofd, en veranderen niet van grootte of plek. Jaren later zitten ze nog steeds op precies dezelfde plek. Het andere bijzondere is dat het erg therapieresistent is, het reageert niet of nauwelijks op lokale corticosteroïden of andere lokale of systemische ontstekingsremmers.

Facial discoid dermatosis is zeldzaam, of onder-gerapporteerd. Het is voor het eerst beschreven door Ko et al in 2010. Sindsdien zijn er verschillende case reports gepubliceerd. De oorzaak is onbekend. Er is een case report waarbij later pityriasis rubra pilaris ontstond, leidend tot de hypothese dat het een variant van PRP zou kunnen zijn, maar in de andere case reports wordt die relatie niet bevestigd.

DD: seborroïsch eczeem, cutane lupus erythematosus, dermatomycose, atypische psoriasis, verruca planae.

PA:
De histologie is weinig afwijkend en niet specifiek. De epidermis toont enige acanthose, omschreven als psoriatiforme epidermale hyperplasie, en parakeratose. Er kunnen folliculaire pluggen aanwezig zijn. Subepidermaal een gering oppervlakkig perivasculair lymfocytair ontstekingsinfiltraat. In de follikels worden vaak Demodex mijten gezien maar dat is normaal voor deze locatie.

Therapie:
Enkele case-reports beschrijven een gering effect van tacrolimus zalf, lokale corticosteroïden, calcipotriol crème, en calcipotriol/betamethason in combinatie met een lage dosis acitretine oraal. Er is 1 case report waarin effect wordt beschreven van ustekinumab, maar de foto’s in dat artikel zijn niet overtuigend en het wordt niet vergoed voor deze indicatie. Vanuit de gedachte dat het een bijzondere variant van pityriasis rubra pilaris zou kunnen zijn is methotrexaat uit te proberen, mits de patiënt zo ver wil gaan.

De meeste case reports beschrijven dat de aandoening helemaal nergens op reageert. Rahmatulla et al (2021) geven een opsomming van de behandelingen die tot nu toe geprobeerd zijn en waarvan beschreven is in de case-reports dat ze NIET werkten, en die omvat lokale corticosteroïden t/m klasse IV, intralesionale corticosteroïden, vitamine D analogen, calcipotriol / betamethason, antimycotische crèmes (miconazol, ketaconazol), permetrine crème, metronidazol crème, calcineurin inhibitors, tazarotene crème, tretinoïne 0.05% crème, imiquimod crème, systemische behandeling met doxycycline, claritromycine, minocycline, metronidazol, valaciclovir, itraconazol, fluconazol, acitretine, hydroxychloroquine, prednison, methotrexaat, narrow band UVB, pulsed dye laser, en excimer laser.
