NECROLYTIC ACRAL ERYTHEMA home ICD10: L53.8

Necrolytisch acraal erytheem is een zeldzame huidverandering aan de voeten en soms de handen die sterk geassocieerd is met hepatitis C infectie. Het werd voor het eerst beschreven in 1996 door El Darouti en El Ela bij patiënten met een actieve hepatitis C infectie. Het behoort tot de groep necrolytische erythemen waartoe ook necrolytic migratory erythema, acrodermatitis enteropathica, pellagra, en essentiële vetzuren deficiënties (acrodermatitis acidemica) toe behoren. Het onderscheid met deze andere beelden is dat het acraal is, vrijwel alleen aan de voeten en onderbenen, en bijna exclusief gekoppeld is aan een hepatitis veroorzaakt door hepatitis C virus. Hepatitis C infectie kan ook andere huidafwijkingen veroorzaken zoals cryoglobulinemie, leukocytoclastische vasculitis, porphyria cutanea tarda, erythema nodosum, erythema multiforme, urticaria, pruritus, malakoplakia, polyarteritis nodosa en misschien ook lichen planus.

Klinisch beeld:
Erythemateuze, soms paarse of bruinige plaques op de voeten, met name op de voetrug en op de bovenzijde van de tenen. Soms ook op de handen. De plaques krijgen later een schilferend oppervlak, de epidermis wordt necrotisch en bladdert er af. Klinisch uit zich dat als schilfering, of oppervlakkige blaarvorming, soms als necrose. In het chronische stadium ontstaan hyperkeratotische plaques. Het kan zich uitbreiden naar de onderbenen; nooit palmair, geen nagelafwijkingen.

DD:
necrolytic migratory erythema, acrodermatitis enteropathica, pellagra, essentiële vetzuren deficiënties, acute stadium van graft vs host disease, acraal erytheem bij cytostatica.

Diagnostiek:
Biopt. Algemeen bloedbeeld, leverenzymen, screening op hepatitis C. Hoewel zink is beschreven als een mogelijke therapie, was het serum zink gehalte in de onderzochte patiënten niet verlaagd.

Histologie:
Psoriatiforme veranderingen, keratinocytnecrose. Hyperkeratose, parakeratose, oppervlakkige keratinocytnecrose, spongiose. Verder teleangiëctasieën, erytrocyten extravasatie, en een perivasculair lymfocytair infiltraat.

Therapie:
Lokale corticosteroïden zijn matig effectief. Het belangrijkste is behandeling van de hepatitis C door de hepatoloog. Interferon alfa wordt beschreven als een effectieve behandeling van necrolytisch acraal erytheem. Ook zinksuppletie is mogelijk effectief. Voor hepatitis C zijn ook nieuwe antivirale middelen op de markt gekomen zoals simeprevir + sofosbuvir, ombitasvir + paritaprevir + ritonavir, en sofosbuvir 400 mg + velpatasvir 100 mg.
R/ interferon alfa-2b, 3 miljoen E 3 x per week.
R/ zinksulfaat capsules à 200 mg ZnSo4, 1-3 dd 1 caps à 200 mg in te nemen 30 minuten voor de maaltijd met een slok vruchtensap. Bijwerkingen: maagklachten, diarree en misselijkheid.
