Perifolliculitis capitis abscedens et suffodiens (synoniem: dissecting folliculitis) is een ernstige destruerende ontsteking uitgaande van de haarfollikels van de hoofdhuid. De aandoening wordt ingedeeld onder de cicatriciële alopecia en veroorzaakt een cosmetisch storende blijvende kaalheid met littekenvorming. Er ontstaan abcessen en met pus gevormde holtes en fistels onder de huid (de term suffodiens komt uit het latijn en betekent ondergraven). De aandoening komt vrijwel alleen voor bij Afrikaanse mannen (AfroCaribbean), in de leeftijdscategorie 18-40 jaar. Het ontstaat door een blokkade van de haarfollikel (folliculaire occlusie) en het kan samen voorkomen met acne conglobata, hidradenitis suppurativa, en sinus pilonidalis. Samen wordt dit de acne tetrade genoemd. Soms zijn er geassocieerde gewrichtsklachten (arthritis, spondylartritis, sternocostoclavicular hyperostosis, SAPHO syndroom), en er bestaat ook een associatie met het keratitis-ichthyosis-deafness syndroom.

De oorzaak is niet precies bekend, het waarschijnlijke mechanisme is dat de afvoergang van de haarfollikel geblokkeerd raakt en dat het haarzakje na verloop van tijd ruptureert waarna de inhoud (keratine, talg, bacteriën) een abcederende ontsteking veroorzaakt, met secundair holtes- en fistelvorming. Dit mechanisme speelt ook bij hidradenitis suppurativa.

Klinisch beeld:
Abcessen, pustels, pijnlijke zwellingen en nodi, littekenvorming, soms keloid, cicatriciële alopecia, fluctuerende ronde en langwerpige holten gevuld met pus, talg en keratine, enkele millimeters tot centimeters groot. Meestal aan de achterzijde van het hoofd (occipitaal) of bovenop (vertex). De met vervloeid materiaal gevulde holten (bulten) zijn karakteristiek voor perifolliculitis capitis abscedens et suffodiens. Ook bij dissecting folliculitis kunnen tufts aanwezig zijn (plukjes haar, kwastjes, meerdere haren die als een kwastje uit 1 opening lijken te komen). Soms is tevens acne keloidalis nuchae aanwezig. Complicaties: infectie, perioden met koorts, pijn, malaise en lymfadenopathie, een enkele keer is een plaveiselcarcinoom voorgekomen.

DD: cicatriciële alopecia (scarring alopecia nno), pseudopelade van Brocq, folliculitis decalvans, tufted folliculitis, CDLE, SLE, tinea capitis, acne keloidalis nuchae, folliculotrope mycosis fungoides (alopecia mucinosa). Pseudopelade van Brocq is een resttoestand zonder ontstekingsverschijnselen. Bij folliculitis decalvans ziet men pustels en tufts, maar niet de holten en fistelvorming. Tufted folliculitis beschouwen sommigen als synoniem aan folliculitis decalvans, en anderen zien het als een entiteit. Zie verder onder cicatriciële alopecia (alopecia atrophicans).

PA:
Diep reikende perifolliculaire infiltraten van lymfocyten, histiocyten, neutrofielen, plasmacellen en reuscellen, uitbreidend in reticulaire dermis en subcutis als abcessen en (suppuratieve) granulomen. Vorming van sinussen, bekleed door plaveiselepitheel en omgeven door granulatieweefsel en oedeem.

Therapie:
R/ antibiotica (tetracyclinen): doxycycline 1 dd 100-200 mg, minocycline 1 dd 100 mg, tetracycline 4 dd 250-500 mg.
R/ clindamycine 2 dd 300 mg + rifampicine 2 dd 300 mg.
R/ clindamycine als monotherapie 2-3 dd 300 mg.
R/ Triamcinolon salicylzuur oplossing 2% FNA (in alcohol 70%), 3 keer per week aanbrengen op de hoofdhuid.
R/ isotretinoïne 0.75-1 mg/kg gedurende 4-10 maanden. Of een lagere dosis (20-30 mg per dag voor langere tijd).
R/ intralesionale corticosteroïden.
R/ infliximab 5 mg/kg per 2 maanden of adalimumab 40 mg per 2 weken. Biologicals worden echter niet vergoed voor deze indicatie.
R/ adalimumab 40 mg i.m. om de week of elke week (hidradenitis suppurativa schema). Wordt niet vergoed voor deze indicatie.
R/ isotretinoïne 0.75-1 mg/kg + rifampicine 2 dd 300 mg; isotretinoïn + dapson.
R/ isotretinoïne 1 mg/kg + antibiotica op basis van kweek en resistentiebepaling + prednison 0.5-1 mg/kg.
R/ zinksulfaat 2-3 dd 400 mg (case report) of zinkgluconaat 1 dd 50 mg.
R/ metronidazol 3 dd 500 mg.
R/ systemische corticosteroïden 1 dd 10-20 mg.
R/ colchicine.
R/ dapson 1 dd 100-200 mg.
R/ dapson 1 dd 100-200 mg + zinkgluconaat 1 dd 50 mg + intralesionale corticosteroïden.

Chirurgisch ingrijpen:
mes Incisie en drainage.
mes Laser ablatie.
mes Chirurgisch verwijderen van abcesholten en fistels (sequentiële excisies, holten in etappes verwijderen d.m.v. grote ovalen, daarna de huid weer de tijd geven om op te rekken).
mes Complete verwijdering van de hoofdhuid gevolgd door split skingraft (therapeutisch scalperen).
mes Preventief verwijderen van alle haren d.m.v. laser epilatie of röntgen bestraling of combinaties daarvan.
